Verify trails for three benefits
The following has been my basic email to Director Jeffries


Dear AZDES Director Timothy Jeffries: 

Would you please answer some questions about your  "Request for information" letter dated June 3, 2014? On page 2 it says I received benefits from 01/26/2014 through 02/25/2014:

"Social Security Retirement, Survivors and Windows Benefits received by Anthony OConnell from 01/26/2014 through 02/25/2014"

"Social Security Disability Income received by Anthony OConnell from 01/26//2014 through 02/25/2014"

"Veterans Administration (VA) Aid received by Anthony OConnell from 01/26//2014 through 02/25/2014"

Comments:
1)  It took me a while to remember that I received one government check ($147.00?) with no explanation that may have been around this time. I cashed it without making a copy. I would like to find out if it was for the 01/26/2014 through 02/25/2014 items described above. 

2) Was there a payment from each of these three sources?  If so, what were the amounts?  How and when were they paid? Has AZDES or AHCCCS ever sent me a check? If so when and what for?

3) What was the justification for starting the benefits?  Please send me copies of the documents that would explain why these benefits were started on 01/26/2014.  What was the justification for stopping the benefits?  Please send me copies of the documents that would explain why these benefits were stopped on 02/25/2014.

4) Your letter asks that I fill out forms for the AZDES, the AHCCCS, and the VA. I don't know what the AZDES, the AHCCCS, and the VA, have in their records about me.  Why are you asking me? What do you have in your records about the three benefits from 01/26/2014 through 02/25/2014?

5) My annual income in 2014 was $12,768.00.  I received $6,684.00 from social security and $6,084.00 from a VA "economic" pension. This VA pension is for veteran's who live below the poverty line. It brings a veteran's income up to the poverty line. The poverty line for me is $12,768.00. What part of my income would the following from your page 1 apply to?

You must give us this information no later than 06/18/2015. There are several ways for you to give us the information we are asking for below:
1. Log in to your Health-e-Arizona Plus account at 'www.healthearizonaplus.gov' to scan, upload, or email information. or
2. Fax your information using the attached fax coversheet to 1-888-372-8777. The fax coversheet has a barcode that identifies your case. or
3. Mail your information to: AHCCCS. 801 E. Jefferson Street. MD 3800, Phoenix, AZ 85034 If you cannot get this information to us by the due date you may be able to get more time. You can request more time by calling the number above. If we do not get the information and we do not hear from you, we will deny your application or stop/change your benefits."

Please send me your documentation for the three benefits you say I received. Please respond.

Can anyone get Director Jeffries to answer the questions? I believe my comment 3 is the best question because people understand that there should be a written explanation for starting a benefit and a written explaination for stopping a benefit. And the explanations should have been sent to the recepient of the benefit:

If the actual paper trails can't be exposed something is wrong. Director Jeffries has virtually unlimited resources and power to expose the actual trails and I have virtually none. Please keep in mind what the "Request for Information" required of me -
